1Members who are under age 65 with unmarried children ages 15 days through 21. Your Attained
Age for insurance purposes will be the same as your actual age beginning on the first day of the month
following your birthday. 15 days through under the age of 27 can apply for $5,000 in dependent coverage.
2Critical Illness Insurance Plan coverage issued on a guaranteed acceptance basis is subject
to a pre-existing conditions limitation, which means no benefits will be payable for a condition for which
the insured received medical treatment, care, advice, or medication within the 12 consecutive-month period
before the effective date of guaranteed acceptance coverage.
3Rates for age 60–74 are renewal rates only. You can apply for new or additional coverage only
up to age 60. Beginning at age 65, benefits are paid for a maximum of two years. Premium Credit discount
shares favorable financial results with Plan participants; the reduction is not guaranteed but reevaluated
semi-annually. To be eligible to apply for the Disability Income Protection Plan, you must be actively
working 20 hours per week and not in the active, full-time service of the military. There is a 25% increase
to the Standard rate if the applicant uses tobacco. If the applicant does not qualify for Standard rates, it
may be possible to qualify for one of the two additional substandard rate classes.
